When the Story Doesn’t Match the Evidence
A patient presented a valid prescription for one vial of Ferinject® (ferric carboxymaltose injection). The medication was dispensed and supplied without issue. Within approximately one minute of leaving the pharmacy, the patient returned claiming the vial had been supplied broken and requested a replacement. Investigation of the incident, including review of CCTV footage, ultimately revealed that the medication had been dropped after leaving the pharmacy. The case highlighted the importance of evidence-based decision making, documentation, and maintaining professionalism during patient disputes.
